Whispering Madness and this guy. Couple it with any and all other similar effects, which all happen to be in Grixis colors,and then throw in massive discard like Rakdos's Return, and your opponents won't be lasting much longer.

When you're trying to play EDH Mill, you can't forget card draw. Cards like Wheel of Fortune, although sometimes beneficial, can be just as damaging as a general mill spell. A legendary creature like this is exactly what a Draw-Mill deck needs. Underworld Dreams+Howling Mine on a legendary? That's not too shabby.
This is the first commander I actually want to build around. Pushes you to victory both damage and card wise. Beautiful design and art. I squealed in excitement when I saw it.

This guy is just so cool; the art, the flavor, the abilities, everything. I think I've found my new favorite Commander, when I saw his spoiler I immediately started putting a deck together. Lot's of great suggestions already, but I'll throw out a few I haven't seen mentioned yet. Chains of Mephistopheles is great if you have/can afford a copy. Mikokoro, Center of the Sea is awesome, as is Jace, Memory Adept(dat -7). Seizan, Perverter of Truth has obvious synergy. With Nekusar out Nin, the Pain Artist can be used to great effect as a removal tool while also dealing damage to your opponent through the card draw. Gnat Miser and Locust Miser keep your opponents discarding often and activating Megrim. Sire of Insanity is also useful there, and shouldn't hurt you much as you should already have out a Font of Mythos or the like by the time you can play him. Urza's Armor is nice for keeping your Spiteful Visions and Phyrexian Tyranny from burning you as well, and it's great for mirror matches. Also, Bane Alley Broker can be really handy for tucking away key cards you don't have the mana for yet before playing your Wheel sorceries, and you don't really mind losing those spells along with her as they would have been discarded/shuffled back into your deck anyway when you played your Wheel of Fortune. And If you want to lose all your friends, give Nekusar infect via Glistening Oil, Phyresis or Grafted Exoskeleton. Sadly, I value friendship and didn't include those last 3 in my deck, though I'm sure others will be less merciful than I ;)

I have a group mill Grixis colored deck already with cards that make everyone draw, mill, and/or discard; there's plenty of them. Up until now my commander has been Sedris to get back a milled Labratory Maniac in a pinch (its not a very commander reliant deck other than that), but this guy might make a better general-at the very least, he'd be good in the 99.
Now, does the damage he deals for card drawing count as commander damage? <.< Why wasn't he the commander of the decks instead of jeleva jeeeeeeez this guy seems amazing.

I'm a little unimpressed with stappling a Spiteful Visions to a legendary creature, but he quite obviously fills a hole in that there wasn't really a punishing legend in Grixis for such a deck. Crosis, the Purger and Nicol Bolas fill the Howling Mine discard Group Slug archetype, but until now now Grixis legend for the punish-them-for-drawing-and-not-discarding archetype that likes Viseling and Phyrexian Tyranny. Too bad I already settled on making my deck like this blue/red only, but I will look forward to having my deck and a Neuksar deck or two at the same table for some very fast and fun games.
No doubt he's a great commander. Of course, what your opponents need to worry about the most is if you give him infect.
Since the damage for drawing is dealt by Nekusar, enchantments like Corrupted Conscience and Phyresis can be pretty deadly. Cast any kind of wheel effect and people are going to die very quickly.

Why is everyone talking about discard and mill, when Black Vise effects exist? No need to try and fight the massive card advantage you're giving them, just let the massive fistful of cards they're holding do that for you.
On a side note, Nin, the Pain Artist is also great with this strategy. Burn away your opponents' creatures and deal as much damage to their controller.

Why has nobody yet mentioned cerebral vortex? I'm running it as a finisher after things like forced fruition, or chaining it once ive drawn into it with any of the afformentioned Wheel of Fortune style cards littered throughout the comments here.

From nekusar again for a total of 18 minimum damage off of one hand reset card. God forbid they tried to be drawing cards on their own. Stake Jace's archivist, ectact (who's ability can be activated AFTER you cast cerebral vortex, remember), and this becomes a much larger number, all over the board, for about 7-10 mana.

Well, I have this guy as Commander and I can tell you: "Once you play with him, you will love him. Once your opponents will play against you, they will hate him (and will hate you as well :P). At my first multyplayer game (4 players) with him as commander was 1 vs 1 vs 1 vs 1 (I won by sneaky tactics of Nekusar and Wind of fortune(s) cards with help of liliana´s carres and and underworld dreams etc...), on 2nd game, it was 3 vs 1(me :()" He is, on my opinion, one of the most hated generals ever, cause he put automatically on cloak when enters and gives an opportunity to deal lot of damage in single turn. But he is the very fun general to play.

The best part about this guy is your friend's reactions to him. They'll either try to kill you immediately, or they'll try to use you to kill the other players. I once used a Blue Sun's Zenith on one friend to do like 8 damage, then Nekusar friend played price of knowledge. NOTE: Do not play price of knowledge when Nekusar is on the field. The other players will NOT use you, they'll just kill you.

If you're like me and don't want to run counterspells, don't forget your Vedalken Aethermage as tuck protection.
Matchup suggestions: Dissipation Field in the mirror match to repeatedly bounce their punishment cards (better than No Mercy because it hits permanents! and you'll probably just end up shuffling their hand into their library anyway), Blood Oath naming creatures if you just straight up need to deal 21 damage right now against pesky old Maelstrom Wanderer, Leyline of Punishment and Leyline of the Void to make sure your hits can't be undone by recursion and lifegain.
You haven't played this card until you've been drawing 20 a turn in EDH.

Combining this guy with elder mastery in my discard deck. Think about it, for every card they draw, they have to discard two. Throw blue sun's zenith on top of that and maybe a nice liliana's caress and that's usually game.

Aside from ChttrBox's suggestions, if you're playing him as your general instead of Jeleva, look for Wheel and Deal, Wheel of Fate, Windfall, Memory Jar, Dragon Mage, Reforge the Soul, Whispering Madness, Diminishing Returns, Whirlpool Warrior, Winds of Change, TIme Reversal, Molten Psyche, and last but definitely not least, Niv-Mizzet, the Firemind.
Magus of the Jar is also invited. Sakashima the Impostor has a VIP pass. Copy Enchantment on Underworld Dreams for even more pain. And why not Gratuitous Violence while you're at it? Dear goodness, drawing is gonna be awful.
