Fun if you have two Megrims in play. Sure, you probably won then anyway, but I like such chain reactions.

@majinara Sorry to burst your bubble, but it's not a chain reaction. A single Megrim only deals 2 damage when they discard. Even if you have two Megrims out it won't turn into a chain reaction because Pain Magnification states that the opponent must be dealt 3 or more damage from a single source. Each Megrim is a separate source.

A pretty damn good card for black/ red beat-down. Even in 1 v 1 if you get it out early they will probably lose 4-5 cards due to this, and in multiplayer it's even stronger. (Also there is a way to do a chain with this Megrim, Akki Lavarunner, and Chaoslace, but it would be incredibly difficult just to do "target opponent discards their hand and loses 3 life per card" and you would still need something to start the reaction)

Does this count if one makes an opponent discard two cards with megrim?

Makes Blightning even more fun. 3 damage and 3 cards? YES!

Would this trigger with a Font of Mythos or 2 Howling Mines and Underworld Dreams. The ruling is not clear on this as they draw 3 cards on the draw step and Underworld dreams does 1 damage for each card. I'm not sure if this counts as 3 separate triggers or if they would be at the same time.

With Pain Mag and Megrim out, Blightning can hit for 9 damage and three cards for only three mana.
I was going to point out the combo that Esperlord just mentioned, but he beat me to it.
But still, Pain Magnification + Furnace of Rath + Megrim = "Whenever an opponent discards a card, that player discards his or her hand and is dealt 4 damage for each card discarded." Now there's a situation that should tilt the game in your favour.
Considering that this is a Rakdos card, it can also help you achieve hellbent when you take damage. Sadly, a disappointingly small number of cards make hellbent truly seem worth the effort. Anthem of Rakdos, Demonfire and Rakdos Pit Dragon seem the best to me. Gibbering Descent is good too, but only as a combo piece, albeit a very strong combo piece, considering the many black cards that require you or every player to sacrifice something, lose life or discard cards during upkeep.
Red-black is all about one thing and one thing only: hurting people. This card makes sure it happens, and it does a good job of it too.

Note, this card only affects opponents. Bad in that it doesn't let you discard when you take damage, for whatever reason you would want to do that, but good in that it's very effective in multiplayer. Just putting it down and doing nothing will cause at least 6 or 7 cards worth of discard through the course of an FFA EDH game, as everyone attacks the player who just played Vorinclex and your opponents hack it out amongst themselves. It becomes even better still when you're running a lot of 3 power creatures, like Beast tokens from Garruk Wildspeaker or Graveborn tokens from Sek'Kuar, Deathkeeper. In fact, it's almost good enough to be in every EDH deck that's into these colors. It's that good.
Is it me, or this can deal an ungodly amount of damage in multiplayer? Damege doesn't have to be dealt by a source you control, so whenever someone gets hit, they have to discard.

Best to use with run-of-the-mill above the curve creatures. Veteran Brawler, Stormblood Berserker, and other 2-drop 3+s make for a very efficient means to cause rampant discard on curve. Hellspark fits well here for repeatability, and Guul Draz Vampire can be a late-game source of not only damage but repeatable discard
