Probably the best combo-killing card there is. Even if you don't know their deck, you get to look at it and then you're all set if you have another.

The question is: which is better in a mono-black control deck Cranial Extraction or Extirpate?

Well Extirpate is definitely strong in that it costs 3 less to cast, but it can only target the gy, so it should be used when you're making your opponent(s) discard, otherwise the Extraction is better.
Don't overlook Meddling Mage as an alternative though. Although the mage is susceptible to destruction unlike Extirpate and Cranial Extraction, it works in White or Blue Control and will give the opponent 4 more dead draws, unlike Extirpate and Cranial Extraction, which thin them out of the deck.

Perfect if you want to get rid of an annoying Annihilator card.

Fun against battle of the wits. You will always beat them, assuming we're talking just playing 1 mana lands every turn, before they can play their namesake spell.

Some very heavy control decks completely or almost completely rely on that *one* win condition card. This is one of the ways to try to break it. Great sideboard material.

Hey, you're playing a self-mill deck?
I CALL UPON... LABORATORY MANIAC!

Oh, is that a Storm deck?
I DECREE THAT THERE WILL BE NO GRAPESHOTS PLAYED FOR THE DURATION OF THIS DUEL!

You think you're clever with your persist deck?
MELIRA, SHOW YOURSELF!

"hey man! I am so excited! I just added some Wurmcoil Engines to my deck; I think it'll really make my late-game."

"Yeah? Well let's try it out :D"

*grabs mono-black control to ensure no fun will be had*
