A rather weak creature if you consider the two colors in the costs. But the name suits the ability.

CMC for a 5/5 is ok, although it has a harsh drawback, flavoursome but sidelines the card potential.
if the rulings were any different such as;
Whenever Segmented Wurm becomes the target of a spell or ability, counter target spell or ability, put a -1/-1 counter on it.
it would hold truer to its name, but most likely then, at the cost of being unbalanced.
Still a Wurm is a wurm, and theres always a place for my fav MtG monsters in my decks.

I like the art quite a bit, but the card needs to be cheaper.

Quillspike and some cheap targetted ability (like switching lightning greaves between the two) works fairly well, though it's true that there's a lot of easier combos with quillspike.
It would have been cool if it produced red and green 1/1 Wurm Segment tokens...
Though, pretty much anything can be broken. With this one, you can set up an infinite damage combo with Mikaeus, the Unhallowed. Have Pandemonium out, and use something like Visara the Dreadful or Avatar of Woe to kill it. Mikaues Undying will bring it back, the Pandemonium will do 7 damage to your opponent, then your critter killer will destroy it again, which also puts a -1/-1 counter on it, which annihilates itself with the +/+1 from Undying. Since it has no more +1/+1 counter on it when it hits the graveyard, it returns again thanks to Mikaeus' ability.
Okay, you need to add several more card to actually make it infinite, or some other way to repeatedly target it ({0} equip cost equipments, for example) plus a sac engine...
