How would this work with poison counters from a creature with infect? Would it double the poison counters, do the poison counters and follow up with equivalent damage, or do nothing since "damage" wasn't done in the first place...?

Anmael, it would check how much damage was done this turn, regardless of the fact that it was done in the form of poison counters, then cause that player to an equal amount of life.
